Schedule Appointment

If you are interested in setting up an appointment with me, I can be reached at jeremy@oakwoodcounseling.com or through our New Patient Form at The Oakwood Collaborative, found here. Prior to our first appointment, I will be sending along additional paperwork to complete through our online filing system, Simple Practice. You will receive text/email reminders 24 hours before our appointments. My current rate for a 60-minute session is $120. I am in-network with Aetna, Blue Cross Blue Shield, and Scott & White, and I will take care of all of your billing with insurance. If you have out-of-network coverage with another insurance company, our office is happy to talk with them to find out what coverage you do have, and what your cost would be to work with me. If I am unable to see you due to availability or we decide that it is not a good fit, I am more than happy to help you find someone within our practice or the community who is able to work with you.
